Found this on an unrelated search, never saw this pamphlet, on the steelnavel site,

So, I traced it back to this quad-fold:

The APP has some guidelines, and while you may disagree with them (after all, they are only guidelines, and do not apply in every case), it is a place to start, and to reference if you need to.
